68 year old male with fever and giddiness under evaluation

65 year old male came with:
C/O fever since 2 days
C/O giddiness since 2 days.
History of present illness:
Patient was apparently asymptomatic 2 days ago then developed high grade fever since 2 days associated with chills,intermittent subsiding with medication. 
H/O giddiness and fall 1 episode on 8/12/21 morning, no loss of consciousness, not associated with seizures ,patient got up himself and went home.
Patient went to outside hospital for fever and was given IV fluids.
1 more episode of giddiness and fall on 9/12/21 at 4 am when patient went to washroom,no loss of consciousness, not associated with seizures.
Patient reports weakness of right lower limb since 2 months,dragging his left lower limb while walking associated with pedal edema on left side-was evaluated and told to have renal calculi causing pedal edema.
No C/O cough,cold,sob,burning micturition, vomitings,loose stools,rashes.
No H/O malena,body pains,bleeding gums.
No H/O headache,tinnitus.
Past history:
Not a k/c/o DM,HTN,TB,asthma,epilepsy.
Personal history:
Diet-mixed
Appetite-normal
Sleep-adequate
Bowel and bladder movements-stools passing once in 2 days.
General examination:
No pallor,icterus,cyanosis,clubbing,lymphadenopathy.
Left lower limb edema + 
Vitals on admission:
Temperature-98.6 F
PR-84 bpm
RR-18 cpm
BP-170/80 mm Hg
GRBS-270 mg%
Systemic examination:
CNS:
Higher mental functions:
Conscious 
Oriented to time,place and person
Memory intact
Speech-slurred 

Cranial nerve examination:normal

Motor system:

Plantar reflex-right-flexor,left-extensor

Sensory system:normal

Romberg's test:normal
Finger nose test:normal
No nystagmus
